Who May Avail of the Service: Indigent Families Who Are Residents of Compostela Valley

	Step	Applicant/ Client Activity	Office Activity	Duration	Person In Charge	Fees	Forms	Requirements

	1		Distribute Family Data Survey Form to the Provincial Community Relations Officers (PCRO).				Family Data Survey Form (FDSF)
	2		PCROs proceed to their respective assigned barangays and coordinate with the Barangay Captain/any Barangay Official as to the prospective beneficiaries of the program.
	3	Fill-up the Family Data Survey Form	Barangay Officials provide assistance in the filling-up of forms.
	4		Filled-up forms will be submitted in bulk to the PCRO.
	5		PCRO transmit the forms to the Municipal Social Welfare and Development Office (MSWDO) for evaluation and recommendation.
	6		PCRO transmit the same forms to the Provincial Social Welfare and Development Office (PSWDO) for evaluation and endorsement to the Philippine Health Insurance Corporation (PHIC) for the issuance of Philhealth Card.
			Note: PHIC will release the Philhealth Card to PSWDO after 2-3 months upon the submission of FDSF
	7	Attend the scheduled activity and personally receive the card	PSWDO in coordination with PCRO will schedule for the release of Philhealth cards
			For Renewal of Philhealth Cards:
			PCRO will evaluate and validate the status of the card holder whether or not the holder is already employed, has changed marital status, died or has transferred residence.
			If holder does not belong to the category mentioned above, the Philhealth card will be automatically renewed.
